Participation Waiver & Release of Liability

By registering for the Leaside Elite Soccer Club Free Soccer Clinic, I acknowledge and agree to the following:

Soccer is a physical sport that involves inherent risks including, but not limited to, falls, collisions, and other potential injuries. I voluntarily allow my child to participate in this clinic and assume all associated risks.

I hereby release, waive, and discharge Leaside Elite Soccer Club, its owners, directors, coaches, volunteers, partners, and facility providers including Toronto Sportsplex from any and all liability, claims, demands, or causes of action arising from participation in this clinic.

I confirm that my child is physically able to participate in soccer-related activities.

I understand that Leaside Elite Soccer Club reserves the right to remove a participant from the clinic if necessary for safety or conduct reasons.

Photo & Media Release

During the clinic, photos and videos may be taken for promotional, marketing, and social media purposes.

No personal identifying information will be published without consent.

Communication Consent

Code of Conduct

Participants and parents are expected to maintain respectful and appropriate conduct at all times during club activities.

Leaside Elite Soccer Club reserves the right to remove any participant whose behavior is deemed unsafe, disruptive, or inappropriate.
